Even the best furniture may need repairs at times.
Start by checking your invoice (if you have it) to see if you purchased our optional Gold Plan Protection with Uniters. Which is valid for 5 years from your delivery date. Additionally, this warranty will cover most accidents (cuts, tears, spills, stains, and even mechanical issues). If you do have a plan, you can file a claim here.
Be prepared to explain what caused the accident (damage/stain/etc.).
If you did not purchase the optional protection plan and believe this to be a warranty issue, please continue with the form below. Some vendors are very specific in what they require before we can pursue a warranty claim or order parts.
Meanwhile, if you took possession of your product within the last 12 months and it was not sold on clearance or as is, we will make every effort to assist you. Which consist of gathering and providing the necessary information to the vendor. This will ensure that the vendor can fulfill their warranty requirements.
Some warranties cover parts and labor, some just cover parts. Forma Furniture will do what we can to assist you within the manufacturer’s warranty! We just want you to know that some labor costs may not be covered.
Finally, there will be a $99 service fee for any issues addressed outside of manufacturer warranty.
