Travis Garrish
Founder | Owner
Travis Garrish, a California native, has worked in the furniture industry since the very first job he took out of high school. After working at several stores and learning the ins and outs of the ever-changing industry, Travis’s entrepreneurial spirit got the best of him. He pooled all his savings, assets, and all the credit he could muster to open Forma Furniture- which became a reality in 2004. Since the opening of Forma in Fort Collins, the surrounding city has grown from a “quaint town” to a bustling small city and the furniture industry has grown right along with it. Despite this growth- Travis has stayed true to his commitment; delivering the highest quality products by the most well-trained, friendly knowledgeable staff. After nearly 20 years- Forma has expanded to 3 locations as well as an online store and now serves the Front Range as the “go-to” store for premier Colorado modern furniture.
When Travis isn’t working, he likes to spend time with his family and enjoys playing golf. He appreciates the arts and is very active in the community.

Connie is one of the most kind, helpful people in the industry (or in general) that you will ever meet. She is well known for her love of dogs and is sure to always keep a jar of biscuits on her desk for any customer that brings their furry friends while they browse the store.

Bailey is an honest and determined Interior Designer who has been a key part of the Forma Furniture team for six years. With degrees in Interior Merchandising and Fashion Merchandising, she brings a keen eye for style and a practical understanding of how to create spaces that are both beautiful and livable. Known for her thoughtful approach and unwavering dedication to her clients, Bailey takes the time to truly understand their needs, ensuring every design reflects their personality and lifestyle. Her integrity, attention to detail, and strong work ethic make her a trusted partner in creating interiors that feel as good as they look.

Angelique Caruso

With a background in art and merchandising, Angelique blended her creative talents with the furniture industry in 2022. She spent three years designing vignettes for another furniture company before joining Forma, where she continues to bring spaces to life through thoughtful design.
Originally from the Jersey Shore, Angelique’s journey took her through the Adirondacks and eventually to the Colorado mountains—a place she now proudly calls home. Deeply inspired by nature, her surroundings often influence both her professional work and personal art.
When she’s not working, you’ll find her painting, hiking, paddle boarding, or skiing. On quieter days, she’s at home with her cats, cozied up watching scary movies or playing video games.

For nine years, Lynn has been a staple at Forma Furniture. If you have been fortunate enough to walk into the store while he was working, no doubt you remember him. Lynn has the effect of making you feel as though you have been friends for years, from the first time you meet.When he isn’t working, Lynn loves watching baseball (especially the Rockies), golfing, and fishing.

Dean McVay
Manager
Having worked in the furniture industry for over a decade, Dean has an understanding of the industry, trends and design that is impressive. He is looking forward to seeing and helping to improve the interiors of some of the North Denver areas beautiful homes. Dean grew up in the area and is excited to bring our new store to his community. When he isn’t working you can find him on the lake paddle boarding with his wife and kids – but his real claim to fame is that his kids are the CUTEST.
KJ Peters
DESIGN/SALES CONSULTANT
KJ has been in interior design for 8 years, with a love of beautiful and quality home furnishings. She has owned her own business specializing in professional organizing & interior design as well as staging homes and really everything in between! She absolutely love houses, watching design and organizing shows, and playing with floor planner. Connecting with people is her superpower! She is a good listener, empathetic, and a solution minded person. Her personality is probably best described as outgoing, fun, kind, and bubbly. She has a great sense of humor and loves laughing. Most important to her are her four children, three boys and a girl. When she isn’t at work she enjoys lifting heavy weights at the gym, hiking, painting, meditation, walking her dog Flash, and watching horror movies.
Shannon Long
Office Coordinator
Shannon is the heartbeat of the office at Forma Furniture, keeping everything running smoothly with efficiency, warmth, and a keen attention to detail. As Office Coordinator, she manages day-to-day operations with ease—whether it’s supporting the team, or making sure the workspace is welcoming and well-equipped. Shannon’s proactive approach and friendly demeanor make her a go-to person for just about anything, and her behind-the-scenes work plays a crucial role in fostering the positive, collaborative environment that defines life at Forma.

Amanda Johnson Garrish
Operations Manager
Meet Amanda, AKA “Travis’ other half”. Amanda is the glue that holds everything together around here. She is kind, enthusiastic, level-headed, and light-hearted. At work, Amanda loves picking out and ordering the accessories for the Forma stores. Outside of Forma, she loves traveling and spending time with friends and family. Amanda also enjoys snuggling up to watch movies with her French Bulldog Clementine. In fact, she has built up an impressively long movie watchlist- and someday she intends to stop working long enough to actually watch some of them.
Trust me when I say that every business needs its own “Amanda”.
